Abstract: 

Background: Objective estimates of likelihood of death in burn patients would be useful in making treatment plans. Recently burn mortality has been decreased. Investigators were trying to find more integrated system of risk stratification in this group. The acute physiologic and chronic health evaluation (APACHE) III is widely used to predict outcome in intensive care units. The aim of this study is to evaluate the prognostic value of ABSI and APACHE III in burn patients.Materials and Method: The current cross-sectional study was conducted during a six month period in Imam-Reza hospital burn unit. In all patients more than 16 year-old the following variables were recorded. Age, sex, TBSA, percentage of deep burn and mucosal injuries, ABSI score, APACHE score and outcome of 90 patients were analyzed. Mean and standard deviation of scores were calculated. The independent t-test was used to compare the mean scores of the variables. P-value less than 0.05 was considered to be statistically significantResults: Ninety patients enrolled into the study (35 male and 55 female). Seventy cases survived (group 1) and 20 died (group 2). The mean score of ABSI system were significantly different between two groups (p<0.001).In contrast on the basis of APACHE system the differences between two groups were not statistically significant.Conclusion: ABSI is a reliable predictor burn patient’s mortality. The issue is in doubt for APACHE and needs to be more investigated.

Abstract: 

Objectives: This study aims to investigate the relationship between dysphonia SEVERITY INDEX (DSI), speech DISEASE and voice SEVERITY of Parkinson DISEASE (PD) patients. Methods: This was a cross-sectional study, carried out on 45 randomly selected PD patients with the age range of 50 to 75 years. The Hoehn and Yahr scale (H. Y scale) was employed to measure the SEVERITY of PD. To measure the DSI, we required detecting the acoustic features of the voice, such as shimmer (dB), vital capacity (VC), semitone range (STR), and voice onset time (VOT),therefore, the participants were asked to produce the vowel /a/ three times for calculating the variables, i. e., STR, VOT, fundamental frequency (F0), second formant (F2), and shimmer (dB). Their voices were analyzed by the Praat software (version 6. 0. 23). F0 and F2 were utilized to calculate the STR. The VOT was assessed by analyzing the spectrograph of the syllable /pa/. The Kruskal-Wallis test was used to understand the correlation between DSI scores and the stage of PD (Y. H scale),the P value of less than 0. 05 was considered significant. Results: A significant relationship was detected between severe DSI and the stage of PD. Meanwhile, the DSI score was statistically significant compared to the scores of the other four groups (P<0. 05). Discussion: There was a significant relationship between the SEVERITY of voice changes in PD patients by DSI and the stage of their DISEASE. Further studies in this field are needed considering that such information may be of cardinal importance for voice professionals to take early interventions, appropriate to the stage of PD.

Abstract: 

Background and aim: Cardiovascular DISEASEs are the main cause of death worldwide. The myocardial performance INDEX (Tei INDEX) is an accepted method for the evaluation of the systolic and diastolic function of the heart. There is not enough data about the association between the Tei INDEX and the SEVERITY of unstable angina DISEASE. In this study, we evaluated the association between the Tei INDEX and unstable angina. Material and methods: The association between the Tei INDEX measured by two different methods (tissue and continuous Doppler) and the SEVERITY of coronary artery DISEASE in 170 adult patients was evaluated. The Tei INDEX higher than 0.55 in tissue Doppler and 0.4 in continuous Doppler methods were considered as abnormal. Finally, the association between the Tei INDEX and angiography findings was evaluated. Results: The mean age of patients was 58.26±10.62 years. The association between Tei INDEX measured by the two methods mentioned above and the SEVERITY of coronary artery DISEASE was not significant statistically. (p value=0.67 and 0.072 respectively) Conclusion: there is no association between the SEVERITY of coronary artery DISEASE (1-3 vessel DISEASE) and the Tei INDEX in patients with unstable angina.

Abstract: 

Understanding the spatial variation of the flood source area within watersheds as they affect of the flood characteristics at the outlet is an important issue in flood control studies. Determining the flood SEVERITY INDEX in a watershed requires study of hydrogeomorphic properties, recorded rainfall-runoff events and use of mathematical models in the context of the methodology to delineate various watersheds areas with respect to the flood downstream. In this paper, Roodzard watershed was selected as the case study since it has suitable rainfall-runoff record. The watershed consists of five tributary subwatershed and three intermediate subwatersheds. ModClark distributed hydrologic model was calibrated in subwatersheds with hydrometric stations. Using HECRAS routing model the whole of the Roodzard watershed model was calibrated at Mashin Hydrometric Station at the outlet. Following the “Unit Flood Response” approach, 2*2 km2 grid squares within the watershed were removed one by one in the simulation process and their effect on the flood peak at the outlet was determined. Such effect was quantified by a flood INDEX and used for preparing the map of “flood SEVERITY”. Furthermore, the profile of flood INDEX along the main stream was plotted in grid-scale as well as for each sub-basin.

Abstract: 

Background: The Emergency SEVERITY INDEX (ESI) may not recognize high-risk patients with heart failure (HF) efficiently. Aim: This study aimed to compare the diagnostic validity and mistriage rates of the ESI plus the Capnometer (Capno) and ESI alone among dyspneic patients with HF. Method: This quasi-experimental group (random assignment) study was conducted within April 2019-February 2020. Patients were randomly assigned to the ESI+Capno and ESI groups. Triage levels, resources used, disposition and door to an electrocardiogram, and physician visit were compared among patients admitted to the Cardiac Care Unit (CCU), the Cardiac Unit (CU), or discharged from the ED. Interobserver agreement (Kappa) was used to assess the reliability of the ESI. Results: In this study, 65 HF patients were assigned to the ESI+Capno (n=36) and ESI (n=29) groups. The undertriage rates were 0% and 10% and the overtriage rates were 10% and 31% in the ESI+Capno and ESI groups, respectively. Sensitivity, specificity, and accuracy to recognize high-risk HF patients were 100%, 60%, and 90% for the ESI+Capno group and 62. 5%, 42. 86%, and 48. 36% for the ESI group. Implications for Practice: The addition of Capno to the ESI increased the validity of triage decisions to recognize high-risk HF patients, compared to the ESI alone. It is recommended that decisions regarding triage HF patients be made after that an End-tidal Co2 is considered into the decisionmaking process.
